The information we collect is only shared with staff members within the office for the purpose of facilitating your care. You have the right to access, update, or delete your personal information at any time.
HIPPA NOTICE OF PRIVACY AND PATIENT RIGHTS: Information revealed is considered confidential and may not be revealed without your consent. The following are the only exceptions. Counseling staff is required by law to break confidentiality for the following reasons: (1) Staff determines you are in danger of hurting yourself, or others (suicidal, or homicidal); (2) There is a reasonable suspicion of abuse/neglect against a minor child/youth, or elderly person, or a vulnerable dependent adult; (3) If counseling staff is ordered by the court to disclose information; or (4) If you direct counseling staff to release information. A copy of the Patient Rights from LCAWC which includes information about your rights to privacy will be issued at your initial appointment. LCAWC’s goals are to ensure your privacy is upheld and to deliver care in the best possible manner.
