We do not sell, trade, or otherwise transfer your personal information to third parties for marketing or commercial purposes. The information we collect is only shared with staff members within the office for the purpose of facilitating your care. You have the right to access, update, or delete your personal information at any time.

HIPPA NOTICE OF PRIVACY AND PATIENT RIGHTS: Information revealed is considered confidential and may not be revealed without your consent. The following are the only exceptions. Counseling staff is required by law to break confidentiality for the following reasons: (1) Staff determines you are in danger of hurting yourself, or others (suicidal, or homicidal); (2) There is a reasonable suspicion of abuse/neglect against a minor child/youth, or elderly person, or a vulnerable dependent adult; (3) If counseling staff is ordered by the court to disclose information; or (4) If you direct counseling staff to release information. A copy of the Patient Rights from LCAWC which includes information about your rights to privacy will be issued at your initial appointment. LCAWC’s goals are to ensure your privacy is upheld and to deliver care in the best possible manner.

5. Limitation of Liability: In no event shall the service provider be liable for any indirect, incidental, special, consequential, or punitive damages, arising out of or in connection with your use of the service.
6. Indemnification: You agree to indemnify and hold harmless the service provider, its affiliates, officers, directors, employees, and agents, from and against any and all claims, liabilities, damages, losses, or expenses, including reasonable attorneys’ fees and costs, arising out of or in any way connected with your use of the service or violation of these terms.
7. Governing Law: These terms shall be governed by and construed in accordance with the laws of Louisiana Dept of Health, without regard to its conflict of law provisions.
8. Changes to Terms: We reserve the right to modify or replace these terms at any time. Your continued use of the service after any such changes constitutes your acceptance of the new terms.
